Although we do offer same-day appointments when available, we encourage making appointments as far in advance as possible. Our friendly staff will be happy to answer any questions you have regarding scheduling and payment arrangements.

We accept Medicare ( and secondary) and most other insurances including the following companies:

Blue Cross Blue Shield, Cigna Healthcare, Tricare, Humana, Aetna, 

We do not accept Medicaid.  Self pay rates are available for those without insurance.

Our office is located at 900 SE Ocean Blvd, Stuart FL 34994 in Suite # 330 in the "D" building.  We are on the corner (near traffic light) of Palm Beach Road and SE Ocean Blvd. only a few miles east of downtown Stuart.  We are in the middle of the complex on the 3rd floor in building D.  You can take the elevator near Quest to the 3rd floor and make a right to the end of the hallway to Suite #330.  Beware of using GPS as it may take you to Hutchinson Island which is too far East - if so please turn back towards downtown on East Ocean Blvd and look for the cross street Palm Beach Road at a light.

 This portal is used to provide a secure way to get documents to our patients including:

• New Patient Intake Forms (please print, fill out and bring to your first appointment).
• Medical Records Release Form (please return signed copy to office).
• Requested records that have been discussed/reviewed with you and your provider.

Once established with our practice, you will receive an email communication with your username and password to login.
